New Patient Information
We want to take this opportunity to welcome you to our team and practice.
We are a comprehensive care professional practice that includes several aspects of your oral health. Due to the fact that we have not had the opportunity to examine your mouth and the associated structures of your head and neck area, your initial appointment will most likely include a comprehensive oral examination and a cancer screen. We will take the necessary radiographs (x-rays) and any other records to determine what your needs may be. We would like to receive any current records from a previous dentist before your appointment, so as to minimize your costs and/or radiation exposure. We will then formulate a treatment plan which will include any necessary work as well as the associated costs for your care.
Below you will find the necessary forms you will need to complete online so we have them in our office at least 24 hours before your appointment or your appointment will need to be rescheduled. If you will be utilizing insurance benefits, we would like to have this information before you arrive for your initial appointment. Once we confirm your policy’s coverage with your insurance company, we will be glad to file all of your claims to help minimize your out-of-pocket expenses.
The day before your appointment and every appointment, you will receive a courtesy confirmation phone call or email of your choice. If you should need to change an appointment we require at least 48 hours’ notice ahead of time, otherwise, we reserve the right to bill you for the missed appointment.
